I'm guessing the penultimate is having a terrorist attack that causes your airplane to crash, but on the way down you fall out of it, and just as a shark is attacking you another surviving passenger kills you just as a tsunami crashes over the whole mess.

And the most likely is that while you're having a beer and a smoke with your McMuffin, you'll crash into a chemical plant because you got high on drugs and were having unprotected sex with a gun-wielding partner who has the bird flu.
