Interesting - your outline basically says the same thing as a report I read on the future of healthcare - patient driven scenarios, insurance drive scenarios and clinicians/healthcare organization driven scenarios - bottom line: individual and corporate access to information (clinical and business) will drive the future, but in a different way based on the perspective (see above).

The ubiquitousness of IT is causing serious paradigm shifts in many markets.
