I ran into a couple of similar issues. At one point I started getting credit card statements sent to my office because my husband at the time was making fraudulent charges on my cards and hiding the bills from me. (This was before everything was on-line.) That address is now one of the "correct" answers to "Where have you lived?"
When he and I took out a mortgage to buy a house, I brought to the closing a passport and a driver's license, both issued to Athena Mylastname, and signed a statement saying, among other things, that I have never gone by any other name. This was true; I did not change my name when I married.
The idiots at the bank decided that I was AthenaHISlast name, reported it to the credit bureaus AND to the IRS, which sent me a nasty letter informing me that I'd reported an incorrect SSN to the bank. My SSN, of course was correct; it was just not attached to anyone named AthenaHislastname.
We were in the process of divorce by then, so the issue resolved itself when the mortgage was paid.
That was 1997. He died in 2010. My credit records still include that name as one I've used in the past.
