Social Security Tax Reporting Question

I just signed up for SS and I also filled out a voluntary tax withholding form where I requested that 13% Federal tax be withheld. At the end of the year do I receive a income statement from SS detailing my withholding amount to give to my tax guy?

see box 5 for income; box 6 for w/h

be sure to check that tax guy inputs w/h.
In my experience not many w/h on SS so it is easy to overlook if you're not careful.
