Tax Question: Date of Acquisition

Twice during 2013 I sold some VG 500 index money. On the 1099B, for one of the transactions it shows an acquisition date of 4/15/2005 and for the other, that box is blank.

I put the money into that fund on 4/15/2005, and since then, all it's received is dividend reinvestments. It still has much more in it than was removed in 2013.

So:

1. Why was the second Date of acquisition blank -- because it was the same as the first, or because I didn't specify specific shares?

2. Can I assume this is a long-term cap gains sale?

3. Should I enter "4/15/2005" or "various" for that second transaction?

Thanks,

Called VG and got the answers:

1. Because it was multiple dates
2. Yes, it states that on a line above the list of transactions.
3. Various
