Year Form 5498 first used?

Hey there...

I have a Traditional IRA I started in 1984 with a 1983 tax year contributions. I contributed most years through 1997. I'm considering rolling some of that TIRA into a Roth. I've been looking at past taxes and paperwork and don't see any 5498 forms for 1983-1985... Does anyone know what year those forms were first used? If they were not used during those years I assume the IRS got the info from financial institutions another way...??

I have a 5498 for 1986 With my name and address but another persons SSN.. (it has a $11969 Rollover IRA contribution) how did I miss that?? yikes.. That was from Dean Witter Reynolds.. what are the chances of getting that straightened out?

I don't have one for 1987 but did not make an IRA contribution that year.

I have 1988 - 1997 IRS 5498 forms...

I also didn't submit all the proper 8606 forms each year. I've downloaded those for tax years 1990 - 1994. I notice in 1995 I started submitting them, but the wrong prior years contributions were entered so I've got new forms for all the rest of the years too. I guess I will just start completely over and submit all new 8606's with the proper values.

I wasn't very organized in my early 20s.. but at least the $29k I put in is up to about 600k...

Any one else have info on the year 5498s were first used?
