Just read a good article on a recent study. Unfortunately, you need an MRI to see how much of your body fat is internal. BMI and how you look are very misleading.

One thing the article does stay is that if you diet yourself thin you may well still retain lots of unseen internal body fat around major organs and unused muscles.
