No Medicare Coverage

Are there any folks out there that are government employees who have never had FICA taken out of their paycheck? If so, and you do not have city\county\state paid health retirement coverage applicable when retiring, how do you plan to fund your healthcare coverage when you retire?

My DW works with three people in the situation of not contributing to medicare. One of them got a part time second job to get enough quarters, one can get it through his wife, and the third is unsure of what to do. He has been advised to go to the State health care exchange. DW has not paid much into social security, but she has been paying medicare.
