Right travel question(s) for retiree health insurer

Right travel question(s) for retiree health insurer

To be clear, my questions will be for my main retiree health insurer (Horizon BCBS). I will be purchasing supplemental travel insurance, but in this case my questions will be for my regular health insurer.

I'm planning a weeklong trip to Puerto Rico in March. I will be purchasing travel insurance, but I am considering going with one of the cheaper options because PR is part of the US, I have direct flights, and medical care will be easily available.

Before I do that, I'm going to call Horizon BCBS and ask them to confirm health coverage while in PR. But are there other questions I should be asking? I'm assuming regular health insurance would not cover evacuation even for health purposes.

My BCBS policy covers emergencies when traveling outside the US, luckily haven't had to use it, I believe they also sell a supplemental policy for more extensive travel coverage.
