A combination of poor planning and one-off higher than normal IRA withdrawals pushed us waaaay over the lower Medicare bracket last year so my SS got whacked.

Question: 2019 will be much lower and back to a reasonable income level (under $170K).

Once my '19 taxes are filed, how soon could I approach SS and ask for a positive IRMAA adjustment?

The return to normal should happen automatically. This form https://www.ssa.gov/forms/ssa-44.pdf

to appeal is not going to affect anything unless you meet one of the named exceptions..........and poor planning and one-of doesn't sound like it qualifies.
The change will only affect you for one yr.....2020, I believe.

It's automatic. Ours has changed each of the last 2 years and is in effect for one year based on your tax return. There's usually a delay in receiving notification on the Part D drug plan, however that may be because we pay direct rather than having that premium withdrawn from the SS payment.
