I feel you were too generous in the tip. A $1 tip would show you are peeved at the server who failed on numerous issues during the meal, and would be so low, the server would remember it.
Good that you wrote about it to the manager.

My name is Deborah, let me first apologize for the service you got Monday night, and also for the meal being wrong.
After talking to my supervisor that was working last night Virginia never said anything to her so she could take care of the issues. On medium rare burgers we have the customer sign a waiver because of the ground beef and e-coli which are available to all servers.
I have taken care of the issues with the server and it should not happen again. I take all problems very serious and I appreciate when customers let me know about them so I can take care of them.
Again I do apologize for your awful service and food not cooked to your liking.
