I believe most insurers won't process the application until 60 days before the date you want the policy to start (at least that's what I found to be the case). The obvious reason for this is that you could develop a "pre-existing condition" over the longer time period. Also, the premiums will likely be higher 10-12 months out (because of your age and general health care inflation). Your broker (or the insurance company) should be able to provide you with current premiums for the different levels of insurance for which you might qualify, depending upon your health, what medications you take, etc.
