Question about Carry-Overs from TLH

If you do tax loss harvesting and have to carry-over excess losses (beyond the $3k allowed), who keeps a record of the carry-over amounts each year? My guess is that our accountant would do this, but what if we switched accountants and forgot to get a record of the carry-over amounts?

Tax software like TurboTax will remember, but assuming you keep a copy for your records, there is a carry over worksheet that shows this.
