A couple of years ago I asked my PCP about the timing of pneumonia shots (65 years old) because I lost a friend to “pneumonia like” symptoms at age 62. My PCP indicated that because I smoked cigarettes as a young adult, he could justify giving me the shot at my age (early 60s). Since I was leaving on vacation the next day I declined the shot for that visit.

Fast forward, I see the latest and greatest PCV shot is now an updated version PCV21 (Capvaxive) which recently came out and was approved in mid-2024. Based on the CDC website below (under routine vaccination)

Adult Immunization Schedule Notes

it looks like I'm now eligible for the PVC21 shot based on my 63 years of age and lack of any prior PCV shots. Instead of getting the shot at my next PCP annual visit, I might just go to my pharmacy and inquire about the cost of getting the PCV21 shot for myself. I like the pharmacy option better as I doubt my PCP office can tell me my actual out-of-pocket cost prior to administering the shot.

I also see DW is recommended to receive an updated PVC21 shot based on her age (>65) and her prior receiving of the PCV23 and PCV13 shots about 10 years ago (our prior PCP recommended that she get the shots early based on her historic mild asthmatic conditions).

I don't remember much discussion on this issue, but I have to believe that many other member of the forum fall into these categories and might have some comments on the new PCV21 shot.

Thanks in advance.

I'm in late 70s and PCP has not recommended the new shot to me.

Well did you do the quiz? PneumoRecs VaxAdvisor

From that link:
"PCV21 contains eight new pneumococcal serotypes not included in PCV15, PCV20, or PPSV23. However, PCV21 doesn’t contain certain pneumococcal serotypes (e.g., serotype 4) included in other pneumococcal vaccines.

In certain adult populations in the Western United States with data (Alaska*, Colorado, New Mexico, Navajo Nation, and Oregon), serotype 4 has caused high percentages (i.e., ≥30%) of invasive pneumococcal disease (IPD). We currently don’t know if this is seen in other parts of the Western United States that don’t routinely monitor IPD data.

Typically, individuals living within these geographic areas who develop serotype 4 IPD are adults aged <65 years with specific underlying conditions or risk factors such as alcoholism, chronic lung disease, cigarette smoking, homelessness, and injection drug use. Importantly, these individuals usually haven’t received a PCV containing serotype 4. In such populations, other recommended pneumococcal vaccines (e.g., PCV20 alone or both PCV15 and PPSV23) are expected to provide broader serotype coverage against locally circulating strains compared to PCV21 alone."

I can't say that makes it the right or wrong shot for you but you could talk to your doctor. I mean, okay I assume you aren't homeless drug user if you posting here but other factors may apply.

We received our second pneumonia vaccine last year, recommended by our PCP. So we have had the PCV20 and PPSV23.
