DW is going on Medicare on 5/1 and our HD health plan will be individual for just me.
This is what I have for a calculation for the maximum we can put in for 2021 - it would be great if someone could check my math and logic,

4/12 * 7200 = 2400
4/12 * 1000 = 333.33
8/12 * 3600 = 2400
12/12 * 1000 = 1000

Total we can contribute for 2021 is 6,133.33

OK, so you are calculating based on 4 months of the family HSA limits, and 8 months of the individual limits?

Since they are essentially the same this year, you can do
4/12*4600 + 4600 = 6,133.33

DH started Medicare in May of last year, so we did the same with 2020 individual limits.
