Ambetter in Indiana coverage?

Getting ready for ACA signup in 2017. It looks like with 2017 pricing, Ambetter from MHS will be our choice for a silver plan as Anthem and MDWise have seen substantially higher increases.

Has anyone here had and utilized one of their EPO plans?

If so, have you had any issues with procedures that have turned out to involve out of network providers, i.e, anesthesiologist, surgeon, etc. that resulted in billing surprises due to non-coverage?

We have no worries about routine health care but are concerned about falling into a no stop loss limit in a major healthcare event that involved hospitalization and surgery, etc. out of our total control.
