Mine is a bit more than yours, but still down almost 40% since it started in 2005. Better than a stick in the eye, but not exactly BTD encouragement. Fortunately, my stash is up enough to cover the shortfall. YMMV

I’ve had mine done 2013. Haven’t calculated the total inflation bite, but like you, we’re primarily living off of investments.

I will collect TRS pension as well about 52% final salary if I take it next year. Curious what if anything is withheld other than federal taxes?

If you will be age 65 or over, and are going with the TRS-Care's custom TRS-Care Medicare Advantage health plan, I assume the monthly premium is deducted. If you will be younger than age 65, and going with the TRS-Care not-Medicare-eligible plan, then I would assume premiums would be deducted for it.
