I will post mine as a reply. I am trying to get some exercise almost every day.
 
DW and I have resolved to resume our daily exercise regimen in order to lose a little weight and, well, live a healthier life. We're shooting for 5-6 days/week of cardio (walking, aerobics/zumba/etc.) along with some intermittent strength training. We just (re)started this week... fingers crossed!

I regularly get in runs and am nowadays adding in planned walks. Also I'm trying to get up and move about the house and yard during the day so the movement is not just a one time burst during the day.

I keep a running/walking/moving log with one spread sheet page for each year. Here is an example (numbers are miles taken from my Apple watch):

I think keeping some sort of log forces one to review the current trend.
