A Question Regarding 2023 HSA Distributions & Tax Prep

2023 was the first year I used my HD healthcare plan qualified HSA for a substantial amount. (a little over $5000 total)

I just finished my 2023 return (H&R Block deluxe + State)
Ended up as I expected (small refunds from both the fed & the state)
Good news — your return looks risk-free!

Was just about to e-file, when I realized that while I had entered my 2023 contribution of $4850, I wasn't asked about any distributions ?

Should I go ahead and file, or dig deeper ?

Yes, you need to report the withdrawal and check a box indicating the the withdrawal was used for qualified medical expenses in order for the withdrawal to be tax-free.

I know OP found this but posting for benefit of others.
 
For benefit of others, what is the form you found?
