My Dad lives in the supposed #1, Crestview/Ft. Walton/Destin (FL panhandle) and while it's a decently priced place to retire with great leisure amenities there's no way it ranks as high as it does for healthcare. Any time he has to have a procedure done it's an hour drive to Pensacola, there's no serious facilities in his area. And even then Pensacola is not exactly known for world-class healthcare, he's had to go up to Birmingham (I think) for heart-related stuff.
