Idearc, Inc. Offering to make up a Full Lot

A family member rec'd an offering to purchase shares in the Verizon spinoff to make up a full lot, 100 shasres. There is a deadline to this offer and they lost the paperwork. I went on the Verizon & Idearc websites to try and find this paperwork and could not. Also tried to find the deadline date to see if it is poast.

If anyone has this info I would appreciate it.
