Coordination of Medical Care

I'm looking for some advice as to how to coordinate my medical care providers.

For background, I had a retinal tear that was repaired overseas. My own ophthalmologist (#1) sent me to a retinal specialist (#2) who agreed the tear was repaired properly, however shortly after the all-clear I had a sudden dimness in the same eye as the tear. Ophthalmologist #2 sent me to doc #3, a neuro-ophthalmologist. #3 had an MRI done and said I have restricted blood flow to the optic nerve. Some Googling suggests that this symptom may well be part of a larger cardio vascular problem. Since both parents had strokes at about my age, this kinda freaks me out. :(

#3, the neuro-ophthalmologist, said go back to #2, since he was the referring doctor. I'm feeling like I'm managing my own care as I get bounced around from doctor to doctor. I guess I could go to my GP (#4) and ask her advice, as she is a generalist and has been watching my overall health.

So, my question is , "how do you coordinate multiple doctors to ensure your overall health is maintained?"
 
I usually check back in with PCP for overview. Your PCP is "supposed" to be your medical co ordinator, but I know my doc is so busy, that I have to be the one that goes back and check in.
In your case, I would go back to #2 opthomologist to find out risk/plan for restricted blood flow to optic nerve and also go to PCP for cardio vascular risk/concerns.
