There's a place where you can record (and keep recording, every day) the fact that you've NOT acquired SARS-CoV-19. You can report you're sick too, but the idea is to keep reporting, sick or not.
It does want your cell phone number, and I didn't put it in there.

I'm not sure how they can use the data because there's going to be a bias in people who choose to visit. But they can watch how many of the regular reporters get disease, and draw conclusions from that, I suppose.
